What you will need for wellness programs
Before we get started I would like you to know that you will need to have some minor pieces of equipment, because ultimately I want you to be in charge of your own health.
You must have the ability to check urine and saliva pHs, blood pressure, blood glucose, and breath rate and breath hold. If you are missing any of the tools needed to check these numbers at least once, you will need to acquire them before I, as your wellness expert, can finish your report.
You will be sent a detailed health questionnaire that must be filled out in full before your first appointment.
You must be a member of the Coalition. There is an annual $20 membership fee and you can cancel at any time.
I will then send you a release and liability agreement in a separate email. You must sign this and send it back to me before you start.
